The Impact of Persistent Reflux of the Below-Knee Great Saphenous Vein on Quality of Life Following Treatment of Reflux of the Above-Knee Great Saphenous Vein

Abstract
Objective: This study aimed to investigate the impact of persistent reflux of the below-knee great saphenous vein (BK-GSV) on quality of life (QOL) following treatment of reflux of the above-knee GSV (AK-GSV) with concomitant stab avulsion of tributary varicosities
.
Methods: From August 2015 to September 2018, 67 limbs with untreated primary varicose vein with reflux in the whole segments of the GSV were included in this study. Patients underwent DUS and assessment of Venous Clinical Severity Score (VCSS) and the Aberdeen varicose vein severity score (AVVSS) before treatment and at 6 and 12 months after treatment.

Results: Reflux in the BK-GSV was resolved in 34 limbs (34/67: 51%) at 6 months and 18 limbs (18/37: 49%) at 12 months following treatment. The percentage improvement in VCSS and AVVSS was 46% and 54% respectively in the group of competent BK-GSV and 58% and 80% respectively in the group of incompetent BK-GSV at 6 months (p=0.52 for VCSS, p=0.28 for AVVSS), and 95% and 87% respectively in the group of competent BK-GSV and 90% and 92% respectively in the group of incompetent BK-GSV at 12 months (p=0.15 for VCSS, p=0.84 for AVVSS).

Conclusions: After treatment of the AK-GSV with concomitant stab avulsion of tributary varicosities, QOL improved in most patients regardless of reflux status of the BK-GSV.
